Eden Health is grateful to work alongside a wide range of organizations throughout Southeast Idaho. We collaborate with assisted living communities throughout Southeast Idaho to support residents, families, caregivers, and facility teams. Many of these partnerships allow residents to receive home health services without leaving the comfort and familiarity of their community. When comfort-focused support becomes appropriate, our Hospice Care team works alongside facility staff and families to ensure compassionate, coordinated care. The following are examples of community partners and are not intended to represent a complete list.
Our partnerships with skilled nursing and transitional care facilities help ensure patients receive coordinated, well-communicated care as they move from a hospital or facility stay back into their own homes. For some patients transitioning home after rehabilitation, Remote Patient Monitoring provides an additional layer of support by helping care teams monitor certain health conditions between visits.
