At Eden Health, caring doesn’t begin and end with the services we provide. It extends into the communities where our patients, families, neighbors, and employees live every day.
Throughout Southeast Idaho, our team is proud to participate in health fairs, educational presentations, senior center activities, community celebrations, volunteer projects, and local events that bring people together. Whether we’re sharing health resources, supporting caregivers, celebrating with seniors, or partnering with local organizations, every interaction reflects our commitment to building healthier, more connected communities. As a 100% employee-owned organization, our commitment to serving Southeast Idaho extends beyond patient care. Learn more about Our Story and the values that inspire everything we do.
We’re grateful for the opportunity to serve the communities of Southeast Idaho—not only through our compassionate Home Health Services, Hospice Care, and innovative Remote Patient Monitoring program, but also through meaningful community involvement that strengthens the places we proudly call home
These moments may seem simple: a conversation over coffee, a game of bingo, a health screening, or a holiday celebration. Still, they help build relationships, reduce isolation, and remind people that they are supported.
Because caring for a community means showing up long before healthcare is needed.
Eden Health proudly serves communities throughout Southeast Idaho, including Pocatello, Chubbuck, Blackfoot, American Falls, Idaho Falls, Rexburg, and the surrounding areas. Our outreach efforts allow us to educate, connect, volunteer, and celebrate alongside the people we have the privilege of serving.

Community health begins with education.
Throughout the year, Eden Health participates in local health fairs and wellness events where individuals and families can learn more about home health, hospice, palliative care, and other resources available throughout Southeast Idaho.
These events provide opportunities to answer questions, offer educational materials, connect with healthcare professionals, and help community members better understand the services available to support them throughout every stage of care.
We believe knowledge empowers individuals and families to make informed healthcare decisions.
Our team regularly visits senior centers and community organizations to provide educational presentations covering topics such as healthy aging, fall prevention, chronic disease management, advance care planning, caregiver resources, hospice education, and available community services.
Just as important as the presentations themselves are the conversations that follow. We value every opportunity to listen, answer questions, and connect with members of our community.
Strong relationships with assisted living communities and skilled nursing facilities help us provide coordinated, compassionate care.
Throughout the year, we enjoy visiting our community partners for appreciation events, educational opportunities, seasonal celebrations, games, music, and resident activities. These visits strengthen the partnerships that allow us to better support residents, staff, patients, and families.
Whether we’re hosting an appreciation lunch or enjoying live music with residents, we’re grateful for every opportunity to spend time together.
Many of the assisted living communities, skilled nursing facilities, and healthcare organizations we work alongside are featured on our Community Partners page, showcasing the trusted relationships we’ve built to improve patient care across Southeast Idaho.
Being involved in our communities means supporting local businesses, organizations, and community leaders.

Some of the most meaningful moments happen outside of a clinical setting.
From holiday celebrations and seasonal events to live music, bingo, appreciation lunches, and community gatherings, our team enjoys creating opportunities for connection, laughter, and companionship.
These events remind us that healthcare is about more than treating illness it’s about supporting quality of life, building relationships, and celebrating the people who make our communities special.
Every event we attend, every partnership we build, and every conversation we share reflects the values that guide Eden Health.
